Purpose of this Notice
This Notice describes how Care Homies, acting as a HIPAA Business Associate, may use and disclose your Protected Health Information (PHI) and your rights regarding that information.
Uses and Disclosures of PHI
We may use and disclose PHI for: (a) service coordination and fulfillment (e.g., reminders, transportation, shopping); (b) payment; (c) operations (quality assessment, audits, training, security); (d) communications with you via HIPAA‑compliant SMS and other channels; and (e) as required by law (public health, health oversight, law enforcement, to avert a serious threat).
Other Uses and Disclosures
Certain uses/disclosures require your written authorization, including most uses for marketing and any sale of PHI. If you authorize, you may revoke in writing at any time, except to the extent already relied upon.
Your Rights
You have the right to: (a) request restrictions; (b) request confidential communications; (c) inspect and get a copy of PHI; (d) request amendments; (e) receive an accounting of disclosures; (f) obtain a paper or electronic copy of this Notice; (g) choose a personal representative. Our Duties
We are required by law to maintain the privacy and security of PHI, provide this Notice, and follow it. We will notify you following a breach of unsecured PHI as required by law.
SMS with Limited PHI
If you opt into SMS, we may include limited PHI necessary to deliver Services. Messages are sent through a HIPAA‑compliant platform; by opting in, you accept the associated risks.
Complaints
You may file a complaint with us at privacy@carehomeys.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate for filing a complaint.
